Walsall Connected Annual Report - Year Four - Partnership working with the NHS

Walsall Connected, in close partnership with the NHS, continues to demonstrate the transformative power of integrated service delivery. This collaboration has been instrumental in enhancing the accessibility, quality and responsiveness of support services across the borough.

As Walsall Manor Hospital evolves its volunteering network, the programme aims to work closely with digital champions to further support and develop Walsall Connected delivery on site.

A total of 3,368 residents have been supported since launch.

Additional benefits of the collaboration with the NHS

Beyond Walsall Connected delivery and support on site for hospital residents, the partnership has created additional digital support for online processes in the hospital and support for accessing free transport for vulnerable adults. This demonstrates additional benefits for Walsall residents and the impact within services.

My Pre Op support

Walsall Connected has linked with services to provide digital support with My Pre Op. Before the Walsall Connected team assisted patients with completion of My Pre Op, compliance was around 85% to 90%. This is now 99% to 100%.

This has supported theatre readiness and allowed for early health screening of patients. Completing My Pre Op before a pre-assessment could potentially reduce theatre cancellations.

Free bus travel support

Funding is provided by the Passenger Service Improvement Programme under the excluded access programme. Eligible people are supported with free transport through Swift technology and the multi-operator bus ticketing scheme. The passenger incentives provide free travel for individuals who may be unable to access transport and, as a result, are increasingly excluded from society and opportunities.

Since the start of the programme between Walsall Connected and the provider, the codes used have supported patients with 7,135 journeys. This provides support to access key health and wellbeing services.
