Walsall Connected Annual Report - Year Four - Enhancing on-site support through Adult Social Care collaboration

Walsall Connected continues to strengthen its partnership with Adult Social Care through the Customer Experience Centre, enabling the delivery of accessible, face-to-face support for residents. A key component of this approach has been the integration of an Initial Intake Senior Customer Advisor within the service model.

The officer is based at Walsall Manor Hospital or in community centres one day per week. The role is centred on early intervention and prevention, providing timely information, guidance and signposting to individuals and families without requiring a formal referral into statutory services.

Demand outcomes

  • 72% of residents supported were helped or signposted, preventing the need for an initial referral for further support.
  • 28% resulted in referrals for occupational therapy, OTA or NCO referrals, ensuring residents received the support they needed without having to make further contact with the Council.

Over the past year, Walsall Connected has also facilitated a series of network forum meetings, bringing together key partner organisations to deliver in-person information and guidance sessions. Contributions included:

  • Salvation Army - Work for All, covering wellbeing and benefits support.
  • Autism West Midlands.
  • Alzheimer’s Society.
  • Adult Social Care.
  • Gro Health.
  • NHS Self Care Management Support.
  • NHS Information and Outreach Service
