Send us your documents and make a payment

You'll need to include copies of the following:

• your completed application form
• two passport sized photographs of yourself, one of which is endorsed as a true likeness of yourself by a suitable person
• a completed disclosure of convictions and declaration form (you can apply online)
• the original accredited licensing qualification certificate
• proof of right to work in the UK (for example a passport, birth certificate, etc.). If you are an EU, EEA or Swiss Citizen living in the UK, you may need to apply to the EU Settlement Scheme
• payment reference number if you have pre paid, or a cheque made payable to 'Walsall Council'

Fees

The fee for a personal licence is £37.00. You can pre pay by debit or credit card (by phone only - 01922 653050 option 2). You will need your reference number to complete the application.
