Joint Strategic Needs Assessment (JSNA)
A Joint Strategic Needs Assessment (JSNA) is how local health economy, local authorities and third sector organisations work together to understand the future health, care and wellbeing needs of their community.
The JSNA aims to support action to improve wellbeing by ensuring that services meet the needs of local people. It is designed to inform and drive future investment priorities and thereby help to plan services more effectively.
Primary care networks (PCNs)
GP practices across England have begun working together with community, mental health, social care, pharmacy, hospital and voluntary services to form PCNs. PCNs aim to improve access, availability and quality of primary care services to local people.
PCNs will support the development of proactive, personalised, coordinated and more integrated health and social care services.
