Complete and send your application form

After you've passed your test and completed the steps above, you should download the application form:

Complete the form, save it, and send it back to us by email.

You'll also need to send us your payment receipts.

If you're not able to download the form, you can email us instead. Tell us your full name, phone number(s) and email address. We will send you a copy of the application form and tell you what you need to do.

Once we receive your application, we will destroy any documents provided for validation.
