Green Square Accord - Hard To Reach Dementia Support Service
The service works with communities and individuals who are from harder to reach communities, to raise awareness of dementia. Pre diagnosis support is also provided on an individual basis for these communities. The hard to reach worker delivers 121 support, presentations and awareness sessions.
