Your personal adviser

Your social worker is responsible for supporting you until you reach the age of 18.

At 17, you’ll be provided with a personal adviser (PA). They will start to work alongside your social worker to support you into your adulthood. PAs are based at the TLC Hub.

Once you’ve talked with them about your wishes and ideas, you’ll draw up a pathway plan together. This will cover the different parts of your life you may need support with.

This could include:

  • education
  • training and employment
  • where you will live
  • your health
  • money
  • other personal support you might need

The amount of support they can offer depends on what you want and your circumstances.

Until you reach 21, your personal adviser will meet up with you least once every two months. They’ll also keep in touch in between visits. 

You have the right to personal adviser support up to the age of 25. You might want this to be more informal, but it's up to you. We'll work with you to find the best way to match your needs and lifestyle.

We'll try to make sure that you keep the same personal adviser where possible.
