Who are we?
To support Walsall’s Adult Social Care Operating Model (OM) and Care Act Local and National agenda by encouraging the use of universal information approaches, early intervention and prevention services giving citizens over the age of 18 a range of services without the intervention of social care funded services and/or delays and diverts citizens from needing ongoing social care and minimise the effect of disability or deterioration.
Client base covers a wide variety of people from diverse backgrounds and health issues, including Learning Disability, Substance Misuse, Sensory Impairment, Mental Health etc.

Raising concerns on behalf of privately funded service users with home care agencies
We provide general housing advice – sharing knowledge of specialist housing and the wider range of housing providers.
Assisting with application and bidding for properties
Provide practical assistance for re-housing
Assisting to claim housing related benefits (rent rebate, council tax, bedroom tax etc)
We work jointly with other professionals across Access Assessment, Health and Community & Voluntary Sector in order to support your needs
Provide cold calls/safe & well checks
Raise safeguarding alerts
Provide access to universal service signposting and support your access to local services to meet your needs
We work closely with health visitors, GP’s, to share information about your current health needs.
Referrals to Community Mental Health Team, Occupational therapy, Physiotherapy Continence service, District nurse etc
We work closely with and support referrals to, fire, ambulance, police services
Refer on for carers assessments and signpost to local services
What do we do?
Our focus is on helping vulnerable people to carry on living in their own homes and their own communities, preventing them from needing more intensive services and giving them the support they need to do this.
