Hospitals and care homes
If you are a health care professional in a hospital, or a member of staff at a care home that we don’t currently work with you can use our online forms to:
- Request a DoLs assessment (Form 1)
- Request a further DoLs authorisation (Form 2)
- Request a review of an existing DoLs authorisation, or tell us about a change in circumstances (Form 10)
If you are a member of staff at a care home we are currently working with you can use your log in details to access the DoLs portal. In the portal you can:
- Request a DoLs assessment (Form 1)
- Request a further DOLs authorisation (Form 2)
- Request a review of an existing DoLs authorisation, or tell us about a change in circumstances (Form 10)

Representatives of a person whose liberty is deprived
Representatives make decisions or act on behalf of the person who’s liberty is deprived (relevant person). Representatives can be a friend or family member. They can sometimes be from an organisation who is paid to be an advocate for the relevant person.
As a representative you can ask for a review of a DoLS authorisation, to make sure that the restrictions in place are still:
- in the best interests of the person you are representing
- necessary and proportionate for the care or treatment they need to receive
You can use our online forms to:
- Consent to act as a representative for a relevant person and share your feedback with us
- Request a review of an existing DoLs authorisation, or tell us about a change in circumstances (Form 10)
